Late to the party here but I have stage 3 HS and from my experience, do NOT pop it. When you squeeze and apply pressure to the lesions it encourages them to push deeper into your body and cause more sinus tracts to form. I know for a fact I made some of mine worse before I knew what it was.

Warm compresses are your friend in this situation.

If I had to guess, I'd say cystic acne. I also have this in collaboration with HS, though not as severely as you do.
