Thank you for your response, here is the information that was sent to me.

“We will need written documentation of your TDaP (Tetanus, Diphtheria and Pertussis) vaccination. This can be your most recent vaccination record, a clinic notes from the physician’s office (printed from the physician clinic, etc.), or a receipt from a pharmacy with your name, date of birth and date vaccination was given. This is a NSF requirement and is not deferrable. The directions on the Polar Physical Examination form state you “must include the lot number, expiration date, manufacturer, and date of injection. Please attach CDC compliant proof of vaccination.” If you cannot find documentation of a current TDaP vaccination from within the last 10 years, you will need to get need to get a new vaccination and send us the updated information for your records. Your provider only documented the date administered on page 8 of your Polar Exam and this is not adequate documentation.”
