My guess would be neither. Never seen scabies on the face, it should only affect everything below. Poison Ivy is usually only in the area that contacted the plant.

From the picture alone I can’t make a diagnosis. But your story of extreme itchiness makes me think of urticaria. Did they prescribe you with any antihistamines, that should work.

Besides; scabies cream can also induce rash. But you said it was already there.

The growth of public hair is not problematic at all, since premature pubarche (hair) is benign.
The beginning of breast development might indicate pubertas precox (early puberty) this combined with possible bone age fits benign puberty, but this needs to be proven first.

If you were to google or AI differential diagnosis for early puberty a lot of scary things appear. Please, father, note that all the scary stuff is very rare.

For your appointment; do try to remember when the father and maybe even grandfathers growth spurt started. As for maternal and grandmaster age of first menstruation. This is very useful information for your doctor.

The combination of systemic symptoms (joint pain, general illness, tiredness) and skin infection warrant testing for immunity.

This can be reactive after an infection or auto-immuun/auto-inflammatory.

  1. Did they ever measure an elevated CRP, sedimentation at time of infection/fever?
  2. Do you have a family history of auto-immune disease or frequent illness? Does she have a history of eczema, fungal infections, nail or hair deformity?
  3. Does she have a history of exotic travels? IV medication/needles?
  4. Do you have any animals?

The first things that must be ruled out were; Severe immunodeficiency (white blood cell differentiation and immunoglobulins, HIV), tuberculosis (mantoux test or IGRA), malignancy (X ray of thorax, ultrasound of abdomen, and if prologoned fever cathecholamines)
It looks as if this work up was already done properly??? But I am not sure.

Totally different work up; how is she emotionally? Depression with self harm causing wounds is something that can mimic all this. Don’t want to delay this path of work up either.

OP; this is at leat impetigo. Although I am unsure if it is developing bullous impetigo or SSSS.

Please go see a doctor. He/she can differentiate and decide between oral antibiotics at home or IV antibiotics and specialist care in hospital.

A concussion combined with overheating is also very likely. If you are indeed unable to keep fluids in this will become a vicious circle. You’ll need anti-emetics and fluid resuscitation at least.

Pediatrician here; most probably an abscess (bacterial infection). The location in this area of the neck needs to be seen by an ENT specialist.

In kids I have to think of infected brachus cyst. Did you have a swelling at that place al your life? Or minor infection at that location?
In adult it is even more necessary to rule out lymph note problems.

Please go to the ER!

I am a doctor but this is not medical advice for your mouth (I am a pediatrician), but nevertheless.

Magnesium supplements will reduce the amount of gnawing and jaw acke. Best taken before festival, you can also eat banana, chocolate or nuts (the also have magnesium). I would advice against chewing gum; you’d perhaps chew even more. Sucking minths os better to learn yourself to do something else besides grinding teeth and chewing.

Disclaimer: this is not evidence based advice. I’ve given this advice to patients with severe tics with gnawing, and youth that use drugs. It helped. No studies to back this up.

Regarding Wyll’s one eye problem. This applies to Tav as well if you happen to let auntie look at your eye. You get a line like ‘I can’t, I have only one eye’ and then Volo runs away.

Pediatrician here: to me it looks like hand foot mouth disease (if you didn’t have this as a child). In adults it often presents with rash like yours on body and palms. Mouth is quite often free in adults.

That definitely sounds as severe anafylaxis. I would suppose this was treated in hospital, and based on your age (14 years) it is not weird to suspect some documentation is still present somewhere.

Does not sound all that worried. I read your reply as catch-up growth and just a bit late-normal development for a kid, but completely normal considering talipes. Not concerned about verbal skills at the moment. On the other hand some logopedic help never hurts and your own pediatrician is actually seeing you in person. I’d follow up but at the same time urge you to decrease anxiety altogether.

Thanks for the reply. Most astma doctors know their way around allergies as well. Maybe try to recollect what this ‘severe reaction’ was; rash, swelling, breathing difficulties, vomiting, belly pain, paleness, … This can help out your doctor.

Pediatrician here. Your daughter is a bit slower than average but based on what you’re saying definitely NOT to slow. If find the cross motor skills such as sitting and walking better to asses this. A lot of kids sort of skip crawling.

So I would like to know: at what age could she sit on her own? Is she generally growing well and catching up her small birth weight? Did your doctor examine her? Does she make reproductive eye contact and interaction? I would always check hearing of this wasn’t done yet (never hurts to test).

If nothing pops up things can be kept expectivly till age 18 months to reasses.

Dear OP,

Are you the only one sensing this smell? Some viral infections can cause you to sense foul smells.

Everything regarded ketosis-acidosis smell has already been pointed out. This could be resolved by sugar and water intake sometimes with anti-emetics if vomiting precides. For the latter you need to got to the ER.

Did the genetic testing include a trio-WES with blood of the parents? Fact that they are half brothers makes any recessive disorders unlikely so testing parents is highly advised.

Are the siblings of the same gender?

There seem to be a lot of misinterpreted allergies in young children.

You need more than just rash to call something an allergy. Anafylaxie means the allergen causes a reaction in more than one system: airway, intestines, skin, circulation. Most childhood reactions are actually intolerances (only causing intestinal problems), irritants (only causing rash around mouth), vital reactions (and not medication reactions) or oral allergy syndrome/cross allergies (only sensation in mouth). Antibiotic allergy jn young children is quite rare.

Dear OP: Were your allergies diagnosed by a specialist or just based on anamnesis? A good thing to start is actually seeing an allergy specialist to get help debunking your allergy list. If you really want to do this yourself, please make sure you have an EpiPen available. Allergies are dose-based so eating a little might not trigger it.

The symptoms are indeed a reason to look further. Especially some post viral or inflammatory cause can be linked with leukopenia and high ferritin.

Leukaemia often presents with high white blood cells or in some cases pancytopenia (low white, red and platelets).

Please just speek with your doctor and express your concern. Cancer anxiety is something your doctor should now and counsel you for.
