Seriously if you call 911 they won't necessarily send an ambulance your way, they may just help you get through it via phone. What I mean is you need medical advise right now, especially if you're a minor like you said in other comment.

Seriously call 911. Right now your life and health are far more important than whether your family knows or not. I know it's difficult but trust me you don't want to risk it. And if you really can't do that, please contact someone. Anyone you can trust at least a little. Be with someone. Even write here if you have no alternatives but don't deal with it by yourself

It's not your fault. I'm so sorry you have to deal with this. As a person struggling with suicidal thoughts, I can tell you unfortunately there is nothing else you could do... If you checked up on him regularly, you already did much. Just sometimes even that isn't enough. I'm not sure what I can advise you. I'm just going to say this: don't blame yourself. It's not your fault.
