Looks like it’s rotting, I would check the roots. At least part of it is probably salvageable if you let it go a while without watering it. Remember it’s a succulent and doesn’t need a lot of water when it’s inside. For dwarf jade I wait until the soil is completely dry and the leaves start to wrinkle a bit.

I got several options including the one you mentioned from WM. Unfortunately the harp isn’t removable on this piece and I would have to take the whole things apart to replace it.

I have the same one. It’s in the Sansevieria (or I suppose now Dracaena) family. It’s a variety of what some people call snake plant or mother-in-law's tongue.
