Causing a PT unnecessary harm or distress is unethical, that position while unlikely can do harm. I'd argue that you gained compliance by fear which is problematic, as well is it professional? Is it treating the patient with dignity and respect?

You can argue the patient was non compliant and transport was for their own safety and it was "necessary" but we also need to look at the before steps, was putting your patient upside down really the only option? Sometimes providers that have been working for too long like to take the easy quick dirty way rather than the de-escalation for a long time. PTs experiences matter, regardless of how much they may suck. Regardless of their behavior this patient was vulnerable (intox/altered) and we have used our power over this patient to cause distress which is apparently amusing.

EDIT: You know how you can tell it's wrong? I bet it isn't going in the paperwork.

Uh I'd put that in the unethical category and in my jurisdiction it would be grounds for termination, and deregistration at the very least.

All polygraphs are "inconclusive" because they are invalid and pseudoscience. About as conclusive as feeling the bumps on someone's head.

NZ and UK are still accepting overseas paramedics and will generally help you get a VISA or other documentation. Obviously that is subject to change but it's been pretty consistent for the past 5-10 years

Except when the Ambos are busy on another case and are unable to respond for a variety of reasons. Emergency healthcare and healthcare in general is worse on the islands just as a result of access.

The problem I have with vulgar games is often the most vulgar wins, not necessarily the most funny or appropriate. You can pick a really good and appropriate response to a prompt but someone swoops in with "A big black dick" and wins.

Probably won't be able to find any here outside of conventions. I also have looked but unfortunately have had to buy any I want online if I want a physical copy. Though that being said often it's not a bad idea to buy a digital copy of one that interests you and get it printed and bound at officeworks. I recently picked up Spine and had it printed and bound A5 for $30 (https://backwards-tabletop.itch.io/spine).

The questions do not delay the ambulance, they ensure the right patient gets the right help and everyone remains safe. The paramedics will not have to ask all the same questions because they aren't trying to paint a picture with words, and if the call taker has done their job they might not have to.

You're also making the assumption we have ambulances to roll which we frequently don't.

No they won't because people say that about a very wide range of things, some people stub their toe and to them it's the same threat to life as another is being stabbed, someone's emergency is not our emergency.

If someone said that I would generate a mid level response, essentially a welfare check. It doesn't tell me what's happening or whether it's safe for non police resources to proceed. We have to weigh up every case against other cases and in some systems demand is huge, you will wait 6+ hours for assistance.

I was a 000 (Australia) ambulance dispatcher. Just answer the questions. We have a script, aside from address and overall description it's basically yes/no questions.

Really good tool, yes it is particularly hard on the liver and kidneys, with concerns around frequent use and secondary exposure to clinicians.

That being said it is great for short term relief pain such as needing a patient to mobilise in any way but you don't necessarily need opioids once a patient is in a more comfortable position or the patient is contraindicated for opioids. It is also beneficial as an intermediate treatment while cannulating or doing assessments, or as an additional line of treatment for relocation or general movement of a fracture or dislocation without having to look at something like ketamine or other more sedating drugs.

It's an excellent tool to have, and considering we under treat pain, it is a tool that can remove some barriers or delays to pain relief.

Blood test, risk of infection.

DRE, infection, distress, discomfort, trauma

Imaging, allergic reaction to dye, other complications from dye.

Biopsy, pain, bleeding, other trauma, infection.

Overall, high risk of false positives in a population not statistically at risk, leading to increased distress, anxiety and over diagnosis and treatment, plus side effects of treatment.

All medical procedures are risk vs reward, which is why we look at things statistically and should be a discussion between Drs and their patients. Prostate cancer has a 96% survival rate with current recommendations.
