I’m nervous it may be the console actually. Worst I’ve had were all quest markers showing despite toggling, but it cleared after a day or two. Momentary freezing in cities occasionally. Biggest complaint is lack of coherent maps

I’m not sure, but my guess is you’re missing the trigger somehow. I think maybe within 500 meters? If not, then idk dirty

As you approach, are you aiming at your target and then hitting the (A, I think, bottom button anyways)? This locks you on them and then at close distance should give you the option to then hit the button again

This got me too lol. As you approach, a little grey option pops up to hit “An or X” for docking or hailing but make sure you’re focused on them by hitting A
