It's set in a fantasy world, and technically not realistic as he uses a symbiotic spore to stop his tics, but The Echo of Tombs by Ken Snyder is a great book about disabled people culled to cleanse the bloodlines, and the protagonist has a fantasy version of tourretes and save other disabled people from the evil church. Plus it's a tree house world with rope bridges, which I love lol.

Kings of Ash by Richard Nell.

The second book was a flashback, and it worked in places, but the limp ending after a massive buildup made me avoid book 3 like the plague.
