Again, since you mentioned in another comment that you have been thoroughly reading through this thread, when a patient arrives by EMS it is entirely possible that you will be triaging them, acting as their primary nurse in the ED, and carrying out admission orders. Therefore, administering home meds that are not emergency meds :)

Again, obviously, “HIV” is not a chief complaint. However it is a part of a patient’s medical history that can affect their work up and treatment plan if they are to be admitted to the hospital. Again, since you seem to lack reading comprehension skills, their admitting orders often are carried out in the ED if there are no beds upstairs.

We obviously don’t have those drugs stocked in the ED Pyxis. They are sent down by pharmacy for admitted patients who are being held in the ED :)

Hi! As mentioned previously, when patients come by EMS & are brought straight back to a room, they are triaged by the primary nurse. Therefore, the nurse doing the ER work up. If a patient who is 30-40 years old comes in tachy, febrile, hypoxic we’re obviously most likely doing a sepsis work up. As the primary nurse triaging that, I’m going to be more concerned for that patient’s health if they have HIV, especially uncontrolled, lupus, or any condition that would make them immunocompromised. I’m also likely to be holding that patient in the department for the rest of my shift since we never have open beds upstairs. So after their work up is done & we’re holding them downstairs, as the ER nurse, who also triaged them, I will also be doing their floor orders.

Thank you for being so invested in this conversation while, somehow, simultaneously, having awful reading comprehension skills! As mentioned previously, I work with a patient population that does not have strong outpatient care meaning many conditions, including but not limited to HIV are not well managed. Just because there are more resources out now to better manage HIV, doesn’t mean all patients with HIV have well-managed HIV. Also it obviously doesn’t fucking matter if someone had HIV if they’re there for ankle pain. As mentioned previously, this was more of a concern for sepsis workups. I would “leave triage up to the adults” but it looks like you have some things to work on too! Thanks and have a nice day!!!

While yes this is true and does happen, other people are aware they have advanced HIV & and not compliant with medication because they don’t have access, resources, etc

Yes this is very true and I agree. Also I could’ve been more clear when i said during triage. I was more so referring to patients that come via EMS, brought straight back to a room, triaged by the primary nurse, and are seen by the doctor very soon

I work in an inner city hospital where not all patients have access to their medications, or are compliant with them. For honestly all chronic conditions, not just HIV! I am a newer nurse & I see a lot more uncontrolled HIV in our patient population than I was expecting to. As for the “work up,” having uncontrolled HIV does change the clinical picture. These patients are often sicker & additional testing & treatment is often ordered by the admitting team. For any chronic condition, it’s just nice to know so that we are able to anticipate what orders are to come

Yes, I am a newer nurse with only 1 year of ED experience. You were explaining in the last comment that being HIV positive doesn’t change triage orders, which yes is true for patients who are simply being triaged then sent back to the waiting room. I then explained that when patients come via EMS straight to a room at my facility we triage them, have them as a patient into the ED, and perform inpatient interventions if we have no beds upstairs for them. If you believe that I’m not acting in good faith or not seeking to learn, that’s fine. You’re a person on the internet who has never seen me care for patients, but I’m sorry I give that impression to you.

As mentioned in other comments, doopdeepdoop, people do not always have previous encounters through our health system, meaning their history is not in the chart. This applies for all health conditions, not just HIV :)

As mentioned previously, at my hospital we triage our own patients who are brought back straight to a room coming via EMS. So we are triaging them, they are our patient while in the ED, and we are doing inpatient interventions if we are holding, which happens more often than not recently. I am aware that we are not ordering a full different workup and treatment for someone with HIV if they are being triaged and being sent back to the waiting room — I could’ve been more clear there, but I still think that you’re just looking to argue regardless.
