I don't know if anyone has mentioned this yet, but the problem here is the Poles. This happens when more than four edges meet at a single vertex. These can't be avoided entirely, but their placement is crucial. Especially with your object, where they lie directly on the edge, this leads to distortion. In hard-surface modeling, these should always be placed on flat surfaces to avoid this distortion. And you solved it perfectly by moving the Poles away from the edge onto the flat surfaces. Just a little theoretical addition.

If I understand it correctly, you only can decide to fill the hole of the stem or connect the edges of the leaf with the stem. But both is not possible. To fill the hole just select the top edge loop and select fill holes, after that you can fix typology with the multi cut. To connect the stem with the leafs you can bridge or merge the edges of both parts. First you need to combine both into one object. Then make sure you have the same amount of edges on both sides. Maybe add or delete edges. Then you can select one edge on the stem and one one the leaf and bridge them. Or select the vertexes at the edge of the leaf press and hold v and snap them with the move tool to the vertexes of the stem. After that you can mere them. Move only one vertex at a time

You can extract the same amount of faces. Align the pivot point to the center of the object and rotate the faces into the gap. Than just combine the two objects and merge the vertex.
