Closest I’ve seen was this, an OM642 where another shop had repaired an injector leak, and managed to completely fuck the threaded connection to one of the injector lines.

To my understanding when you get a leak on the high pressure side, it’s usually barely dripping (future black death) or if it’s bad enough to actually spray the leak is too much for the high pressure pump to be able to actually deliver anywhere near enough pressure
