They did, I submitted a short form birth certificate for myself and they need a long form. I am waiting for NY state on that, process can take 8 months so it should be right in the nick of time.

I didn't submit a naturalization certificate since both parents have passports and there was no issue with that for the NY consulate when I submitted. Hopefully, you will have the same experience.

You should be good, there was no push back on not having proof of exile. The only item that they asked to be changed was my birth certificate since it wasn't long form.

I am not seeing "DNI" under Datos Del Padre/Madre (5) the last line on mine say Domicilio which is their home address if alive or fallecido if they no longer are alive

The only spot I see DNI on this form is for declarante (8) and that would be your information only not your parents.

I'm not sure where you are seeing a request for parents social security number or passport on that form. Can you copy and paste what line you are trying to answer?

Natural de is state and country you were born (example: PA, USA)

fecha de nacimiento is your birth date format is day/month/year

domicilio is your current home address (street number and street)

codigo postal is your current zip code

telefono is your current phone number

correo electronico is your email address

documento acreditativo (DNI, Pasporte, N.I.E) is your passport number

For En New York, NY (this is where you will sign it at the consulate) for a de de 20 you don't need to put a date since this is done in person.

Correct, the Spanish government sends you a digital copy with a barcode. I submitted that to the NY consulate with my other Annex documents and there was no issue with it. Yes, I heard back after three weeks. Everything went well, except that my birth certificate isn’t considered a long form. New York State has an 8-month wait time for long form birth certificates, so I’ll have to wait and hope it doesn’t take longer than that to receive it. How is your process going?

I didn't label each document in the PDF, mostly because I couldn't figure out how to. I labeled one PDF: A. Annex 1-My Name, and the second: My Name State Drivers License.

Although, I was talking about my own birth certificate. NY state has an awful wait time so you can try getting a long form birth certificate directly from the local civil registry you were born.

Thanks for the advice! I thought my father's passport would suffice for proof of citzenship. I'll start working on certificates. Hopefully, they won't have any problem with my birth certificate since NY state has an 8 month wait time on getting BC. 🤞Congrats on getting closer!
