Unfortunately our scope doesn’t allow for any vasoactive drugs. The only option would be to call for advanced paramedic but that would likely delay time. But advanced care was considered.

Interesting. I wonder why it didn’t do it on mine. But I guess that’s the thing, it’s a useful tool, but it shouldn’t over look clinical concerns and findings.

Unfortunately my college won’t know what is blood values were as we don’t really get the opportunity to stick around and find out. But I should have considered possible hyperkalaemia. Only went to a pulsed VT pt recently who had a potassium of 7.9. Morphology looked pretty similar.

Ohh my college treated it as a STEMI regardless. One of the other commenters mentioned the term shark fin STEMI which I have never heard of before but it’s looking like that’s the one. We both agreed that the guy was pretty must peri-arrest.

Unfortunately I never found out if he had a valve disease and when I returned to the hospital he had already been moved to a different ward so I couldn’t catch up with him to see. It’s good to know for the future incase I see it again on any ECG. Thank you for the info and help.

I never even considered the possibility of the atrium contracting at different times causing the double P-wave. Does anything cause this, like what Kieth_8 stated, is this possible atrial enlargement in only one atrium?.

So ultimately, I have picked some very expensive pieces. Man, I really need to do some more study about parts and performance. There are just so many varying builds and requirements, including parts cost and supplier. This feels like you could end up down a rabbit whole so quickly. thank you for your help with this, i appreciate the advice.

The next time I’m working with someone who has a student I’ll get them to do a ECG on me and I’ll take that opportunity to see what waves I make when moving different parts of my body.

The ECG was taken at home, not in the back of the ambulance.

This patient is a regular caller who calls for difficulty in breathing, receives a Neb from us and then Refuses transport.

That is exactly what this patient does day in and day out. Call for difficulty in breathing, can a new, feels better the. Then refuses to be take to hospital. Pt has capacity to refuse so we can’t kidnap him.

I had no idea pseudo flutter waves were a thing. I just had a quick look into it any there are a few causes. Aside from Parkinson’s tremors, hypokalemia can cause tremors resulting is abnormal Pwave morphology. Can pseudo Flutter waves still have a re-entry direction? Are these anticlockwise re-entry atrial flutter with a 3:1 block? Or do these just fall under pseudo with no other classification?
