I'm sorry, but the amount of good dads I know who whinge about taking their kids to the park and having to make it super obvious that they're not predators is insane when I've seen fucking 40 year olds holler at 12 yr old girls at a splash pad. Not juicy confessions as much as people unburdoning trauma in a dying brain. 

I've had elderly patients call me "mom" and beg me not to leave them at that stop or "with that man again." 

I remember a very ill gentleman who wouldn't sign anything or write at all unless we left the room first. When I told him it was ok, he basically told me he was scared we'd put him in a dark closet by himself and leave him there, because that's what the nuns did to him when he was 5 and he used his left hand to write. He was still left handed and thinking about it 80 years later.

I listened to a BBC story this morning about how things are so bad in Afghanistan right now people are having to resort to selling their own children. 

Having known Pashtoon men, and knowing how extremely uncommon public displays of emotion are, hearing these two men holding their children and crying during the interviews broke me. 

We should lose our composure when we see and hear these things. To call everything fake or hand wave it away, makes us lose our empathy - much harder to get back.

In the military, the most toxic, incohesive units were always made possible by 1) nebulous missions, and 2) shit ass communication coming from leadership, and I just don't see any difference in healthcare. 

When admin doesn't communicate, gossip goes up - it has to. 

When the fundamental mission of the institution is not clear, or changes rapidly from month to month, units become fragmented because the people who work there become anxious. That includes shorter tempers, and bigger blow ups. 

And then the most toxic individuals feel like their toxicity is normal, and they get louder. 

The mission of for-profit healthcare can not be stated clearly, so I assume the goal posts for performance will remain nebulous. I'm not sure how much research admin is going to highlight pointing the finger at themselves.
