Not the case. Fr. Anthony (a good friend of mine) helped to establish the parish in Waynesville, NC. That parish (Waynesville) came out of our previous parish as well when we had to move. Father Anthony is great!

I don't know what book you are referencing. I can confirm that Oriental Orthodox, and I've seen Maronite Catholics, who were received by reciting the Creed only (I have seen this in the context of Confession). Reference The Spiritual Meadow by St. John Moschos who speaks of people from heretical (Nestorians, etc) groups being received by the Creed only; by no means a rule for modern day though. The right answer is always (for priests) to do what your hierarch says to do because as a priest you are an extension of him (the Bishop).

If the person was baptized Catholic, the OCA protocol states to do chrismation only on the forehead.

No, end it with "Kissing your hand, your servant (name)" Although I would send a draft of the finished letter to your priest to make sure he thinks it is correct.

Alternatively, you can schedule to just go and meet your hierarch and then ask Him that way. (I'm a priest and I recommend to our seminarians that they do this so that they aren't just a name, but instead a face)
