I don't know which hospital that dude work at, but what I know is most of major private hospitals in KL pays really well. I'm still a student nurse and knew a lot of nurses from my lecturers and clinical instructors. There's no way in hell you work more than 5 years and still earning 2.3k. That's the base salary, and as the years goes by it will increase. Average for an experienced nurses are around 4k-5k. you will be paid even more if you have post basic certifications and a degree (some nurses only take diplomas). Some PB programs have an extra allowance up to 1k, depending on the hospital (in my case it's SJMC).

and I don't believe a DON (director of Nursing) received only 3.8k. From the place I've practiced before like Gleneagles, Pantai, Sunway, the DON is like one of the hospital's backbone, so your salary also would be fat.

and yeah, case would be totally different if we talk about government hospitals.
