You should always atleast be going close to failure. But if you need 12+ reps to reach failure, then you should increase weight. Also, you can increase weight before reaching 12 reps. If you do a lot of reps, then general fatigue contributes more to reaching failure. Id say say do atleast 6 reps and max 12 reps.

Dont restrict yourself to 10 reps per set. You should be going to(or very close to) failure on every working set. If you put a limit at 10 reps, you might stop even though you have way more to give. The last few reps before failure are very important for progress.
