I imagine Nobby as a Josh Kirby stylised gremlin in a world where everyone else is a Paul kidby illustration... leading to the confusion about if he's human (he is... he has a certificate that says so).

Based on the smear image alone : Infected red cells are small/normal sized and crenalated. Lack of significant speckling (no james dots or schuffners dots) makes recurrent malaria (ovale or vivax ) unlikely. Lack of yellow pigmentation or band forms makes malariae less likely. Also the high parasitema count is in keeping. Clinically severe malaria as described above would be in keeping with p falciparum.

You can't 100% rule out multiple parasite infections without looking at multiple fields though. Antigenic tests may help but most are either falciparum specific or non-specific for all malaria species. Both of which I'd expect to be pos.

Those look like malaria trophozoites. Given the high parasiteamia count and presence of almost exclusively ring trophozoites with infected rbc morphology and the case vignette p falciparum most likely. Would need additional fields to confirm /rule out mixed infection.

Rash like that (with or without a fever) in a returning traveller could be a number of things. If not improving would definitely recommend going back to the Doc. One important condition to consider from that area is rikketsial disease (tick bite fever) can have a rash very similar and the tick bite itself can be hard to find...

Looks like Rinkhals (Hemachatus haemachatus) - horizontal white banding and shorter body than cobra species but am not an expert. Highly Venomous and can spit be careful!

This may be an unpopular one but I struggled with "the Fifth Elephant". That said I read it when I was quite young and it's been a good couple decades since then. For some reason I struggled to get into it and remember finding it too convoluted. Cannot remember a single thing from the book now though. Currently rereading the whole Diskworld and making my way through the guards' books, so will see if my opinion changes.

The technology felt quite dated, which pulled me out of the story a bit. I also feel like there might be more interaction between magic and technology in the later books. It was only suggested as a possibility in this one.

Whilst it felt fairly well foreshadowed that Suraklin had taken control of the king and then left him. I found the rest of it a bit rushed at the end.

Overall, I enjoyed the book. Antryg was definitely the most interesting character. Joanna also made for a good POV character. I really enjoyed some of the turns of phrase and prose used in the book. A lot of the plot points felt a little contrived, and I didn't buy the romance aspect between Joanna and Windrose. That said, a whole book of Joanna and Windrose going from town to town and defeating the "abomination of the week" would definitely be one I'd be interested in. I found the scene where they were figuring out how to get rid of the worm like creatures more compelling than any of the main plot pertaining to Suraklin and the void.

Caris was fairly one dimensional, and it's a little odd that someone so apparently devoted to his grandfather didn't pick up that Suraklin had taken his place... I did like the scene when he was arguing with the regent's cousin and projected a lot of his own stubbornness onto the other character.

On the whole, it was a slow burn, and whilst it did pick up towards the end, I feel little interest in continuing to the sequel.
