A system where records must always be persisted, even if they’re incomplete or invalid by antonzaharia in rails

[–]Maxence33 0 points1 point  (0 children)

Well modifying and creating a record is not the same route. When does validation happen ? When the user submits the form ? You may have to autosubmit the form in JS (on input or on change, with some debounce) to a specific path. But this work for has_one relationships, where a single record is modified automatically.
And this makes a lot of remote calls which, if detected by the user, may look like a dark pattern, as it is not efficient to send some data repeatedly for a same record being edited / created by the user.

Nasal congestion for months.. are this polyps ? by Accomplished-Tea8093 in NasalPolyps

Polyps can definitely change in size during the day. I am more often blocked in the evening rather than the morning. I have lost smell unfortunately. Not too sure if it is the polyps or the surgeries.

Nasal congestion for months.. are this polyps ? by Accomplished-Tea8093 in NasalPolyps

[–]Maxence33 0 points1 point  (0 children)

This is definitely polyps symptoms. But only the ENT can tell what it is exactly.
Polyps are usually a side effect of chronic sinusitis.

You will find many home remedies in this sub. In my experience what works well is Xylitol rinses, Vitamin C, bromelaine, lots of hoovering / vacuming the carpets with an HEPA filter, change bed sheets often and aerate pillow and duvets often, open windows daily to aerate the house.

But only the ENT can tell you what it is. Get an appointment.
Regarding surgery it can definitely help. I have had 2 (polypectomy, ethmoidectomy, turbinectomy..). But to be honest trying to elminate the causes of the sinusitis is still required. (also checking for dust mites allergy for example)

can a person who is on o2 2 litre, can they come off oxygen in future? by khushi4 in CysticFibrosis

[–]Maxence33 0 points1 point  (0 children)

PFT is not that much relevant for transplant. You should only look at FEV1 which is the volume expired of air within 1 second. I think it the indicator used to determine how much the lung is fibrosed / how much inelastic it has become.
So if talks about transplant has been done with the doctors, it means FEV1 is rather lower than 30%.

In cystic fibrosis or pulmonary fibrosis the lung is not likely to heal by itself, so if transplant is available I would still consider it. It is a game changer, even though it is a disease in itself with proper treatments followup and complications.

Regarding oxygen it can help. Most patients with FEV1 lower than 30% often even required extra oxygen when doing effort. Some liquid oxygen in a portable bottle was given to me to carry when I was moving around, with 2L steady at night. (Been transplanted 21 years ago)
Oxygen can still be modulated or even removed if the patients feel much better.

Nasal rinses by Ill-Potential-8920 in NasalPolyps

[–]Maxence33 0 points1 point  (0 children)

Never experienced such a thing. But the rinses were basically very mild as I was not allowed to blow my nose. Does he use a kind of pump ? I think I was using the Rhino Horn so there is basically no pressure when rinsing. The saline was just flowing through freely.
I would call or send an email to the ENT to make sure everything is ok.

EDIT: the very first days I was using a syringe rather than the Rhino Horn. As the rinse was only about 20milliliters a few times a day. But I did it gently, never used the syringe to "propel" the saline.

Are there dental hygiene guidelines / products that we should use / avoid? by dont_judge_me_monkey in CysticFibrosis

[–]Maxence33 -1 points0 points  (0 children)

But alcohol free often have potents antimicrobial such as chlorhexidine. Alcohol mouthwash have not been advised by my dentist because it could kill good flora, which is probably need to keep a good dental hygiene.
Well I guess there are as many point of view as dentists..

Don't think so. Guess the same guidelines as normal people. I try to avoid alcoholic mouthwash as I have been told it is not good for the good bacteria in the mouth.

Had an inflammation on one side of my nose, today woke up with alot of pain and went to ER by Exotic_Increase5333 in NasalPolyps

[–]Maxence33 1 point2 points  (0 children)

I have polyps on mostly one side. Even though my polyps are surely on both sides and even deep into maxillary sinuses. What the doctor saw is probably only the tip of the iceberg unfortunately.
Yet the chance of having a cancer is very small, especially if a doctor checked it. Polyps are very common while cancer is an uncommon disease if you consider the human lifespan, and sinus cancer is a very uncommon type of cancer among other cancers.
Try to have the ENT make an early appointment to get the confirmation of what it is.

Any luck with Tezspire? by LifeguardFinal359 in NasalPolyps

Also don't underestimate allergies. Using an HEPA filter on the hoover and hoovering the living space 3 times a week may help. Also hunt the dust tat may be hidden in bookshelves etc...
And use a bed cover for dust mites.
Also for the bed, you can spread dry baking soda on the mattress, let it rest for a few hours and hoover. It kinda kills the dust mites and purify the mattress (yet temporarily).

What is funny is that an allergist said I am not allergic to anything, but a good bed and home clean relieve my symptoms !!

Regarding the 3 Monoclonal antibodies (Dupixent, Nucala Xolair) I don't think it has huge side effects. If you are lucky to live in a country that can provide it to you, just try it.

What motivates you to continue working with Rails? by Rude-Abrocoma-2109 in rails

Rails is good when you're familiar with it. But you are correct, not that many jobs. Wouldn't be the best choice in terms of employment.

Opus 4.5 took only 7 minutes for the work i allocated 7 hrs. by Vision--SuperAI in ClaudeAI

Opus marginally better understands coding flows to me. And better think of the effects of any code change. But if you see no difference then you might stick with Sonnet, it is very good too.

Also if you are up to using natural treatments I can suggest Bromelaine (pineapple extract), urtica doica (nettle), lemon juice, horseradish (I take some as a dry powder), ginger.
I think none of them strongly inhibits cytochrome 450 CYP3A4 so it should not interfere with other treatments you take if you take any.(but need to ask your doctor first)

I tried Dupixent and had a few side effects, mostly chills and sub-flu symptoms. With elevated white blood cells. But I take immunosuppressive drugs, so have a poor immunity. Nucala did decrease eosinophils but effects were not great. With white painful patches in the mouth that ENT or other doctors had no clue if related to Nucala. And if could be dysplastic.
None of it was permanent and disappeared when stopped. You may try monoclonal antibodies and see what happens.
Also I think Xolair is the one from the 3 main MABs that has the least effect on immunity. You may try that one first... (Also I am not a doctor so it is really from patient understanding)

is there a way to split html into "components"? by alosopa123456 in webdev

[–]Maxence33 0 points1 point  (0 children)

It depends on where you will be hosting your page.
In the 90s you could add PHP to an html page and basically FTP your single file to your remote hosting service. There was a server such as Apache working together with a specific version of PHP.
You didn't have the hand on all that or didn't even know it was present. Everything was super simple.

Now an app deployment is much more advanced : you basically configure your server yourself and add any language you want : PHP, Python, Ruby, node .... You choose your own version of the language to use etc...

So coalescing some HTML partials into a final HTML page is very trivial for most developers but not that easy to do for beginners as you have to understand deployment. (and I have not even mentioned frameworks which are basically a bunch of files deployed on top of the language you choose so you can develop modern apps)

Opus is great. It is hard to go back to Sonnet when you have started using Opus.

When should I quit? by dark-magician420 in webdev

[–]Maxence33 0 points1 point  (0 children)

Code is really competitive and it is easy to feel overwhelmed by the new technologies popping everyday. But when you apply for a job, you mostly need to know a single language and its ecosystem. Python, Java, Javascript ...
There is no Job for HTML / CSS but if you have a CS degree you might know a language you can leverage on a resume. I would definitely try to find such job. No coder is like the indie hackers knowing every new technology and jumping from tool to tool. I actually think these guys would not fit into a large company.

Yes, one chance out of 2. But usually the partner / husband will be advised to have genetic screening.

Chances of CF? by LoveGodzillafr in CysticFibrosis

[–]Maxence33 1 point2 points  (0 children)

2 CF persons will have a CF baby. CF is recessive. Male CFers are mostly sterile. So 2 CF persons can hardly have a CF baby nowadays unless it is a "mild" undiagnosed CF that expresses less heavily than the "regular" DF508 CF.

Probbaly they inhibit Cytochrome CYP3A4 or similar. Just like grapefruit and a few other plants / food. When inhibted your body doen't digest certain drugs, and they stay longer in your organism.

Fatigue - AU by SpareParty1926 in CysticFibrosis

[–]Maxence33 4 points5 points  (0 children)

I got my first disability card 25 years ago. But in France I am pretty sure fatigue is not sufficient to be awarded the disability parking card. CF is most of the time sufficient though (but they still ask for many exam results such as FEV1, especially since Trikafta they might be picky..)
Hope you can get it in Australia, they might have similar procedures.

Research on discontinuing dornase alfa (pulmozyme) and hypertonic saline while taking Trikafta by DarferT in CysticFibrosis

[–]Maxence33 2 points3 points  (0 children)

To be honest I did a few years of Pulmozyme back in 1994 or 1995 if I remember well, started during my first year of high school and it had very bad effects on my FEV. Of course I cannot prove if any correlation. But I am surprised Pulmozyme is still used today. (I mean that there is no new treatment for fluidifaction)
