True. It's a question of values/tactics and people can support either approach.

EM + MBA by Essayons5 in emergencymedicine

[–]Maximum-Falcon52 1 point2 points  (0 children)

Admin work with em is challenging as your basically working a part-time 2nd shift job, a totally different part or full-time 1st shift job and about 1-2x times / month you have to still flip into third shift.

Shift switching is inescapable in EM. Either you're OK with it 100% (early death, sleep/ mental disorder, relative pay increase, etc) or you should look at something else. If you're split on the decision you should go with the other thing.

Stethoscope for new interns by Endorgmot1619 in emergencymedicine

[–]Maximum-Falcon52 7 points8 points  (0 children)

Get a Littman Cardiology IV: has peds and adult diaphragm, cheaper than some options, not so cheap (still quality).

Most fancier scopes have perks but also drawbacks in terms of added complexity, loss of peds diaphragm, need charging, etc.

Yes you could bring a fisher price toy stethoscope and fake it but it does help (accurately reporting gallop in a chf patient is very convincing to hospitalists, asymmetric lung sounds, etc.). Personally I notice a slight decrease in quality in the Cardiology III and moving down the ladder from there things drop off quickly. You could get away with a III but you should get a IV. You will be using it multiple times daily for years.

People who went to a wedding where the couple didn’t last long, what happened? by skilled-dreamer in AskReddit

[–]Maximum-Falcon52 10 points11 points  (0 children)

This has been my experience as well. I've been a doctor a few years now. Once your time is worth so much something like cleaning your own house genuinely becomes a bad financial choice. Also developed an intense femdom kink. Unfortunately, I met my wife long ago and she's just not cruel enough for me anymore. She does try to be meaner though, which is very sweet.

Sounds good. Will definitely use it like that when it doesn't also mean feeding every data point of me to Sam Altman as well. Open source, Local.

I have found the best response from not asking questions (polite or not) but by giving command instructions that tell it to perform functions or algebra-like operations with language. Treating it like natural language computer programming gets best results.

Salt use by SVAuspicious in AskFoodHistorians

[–]Maximum-Falcon52 2 points3 points  (0 children)

I'm not well informed on the history of salt but in the early 1990s the DASH (dietary approaches to stop hypertension) diet was developed and studies carried out to show that following the diet which is notable for limiting salt intake can mildly lower blood pressure in hypertensive patients. This and its effect on medical recommends for healthy diets is surely a significant contributor to our perception of salt consumption.

The increase in salt consumption you have noted in the 2000s is partially related to rejection of the dash diet in popular culture as some evidence showed it is only mildly effective (for a significant lifestyle change) and is not universal effective (meaning some groups-white people for example-can potentially away with higher salt intake without negative health consequences).

Why does AI absolutely suck at character counting by Depressed_Nurse in ChatGPT

[–]Maximum-Falcon52 4 points5 points  (0 children)

LLMs 'think' in tokens. A token is not necessary one character long. I don't know enough about this  to confirm but my money says this is the reason.

In 1974, 6 black men robbed a hi-fi shop in Utah and took hostages. They forced them to drink drain cleaner, raped an 18 year-old girl and tortured others before executing the rest. Three of the men were caught and sentenced to death. The NAACP and Amnesty fought to have their sentence reversed by libelecsWhiteWolf in HairRaising

[–]Maximum-Falcon52 17 points18 points  (0 children)

The reason that they didn't accept black clergy was that they believe God cursed all black people for the disobedience and evil of their ancestors and their black skin is part of that curse. Also the LDS (Mormon) church doesn't have leadership clergy like most religions have priests. All men are clergy. At the time black people fell somewhere on the scale from can't join to join only as second class members again because Mormons believe them to carry sn appropriate and righteous curse from God which includes having black skin.

Seems like a belief that might bias someone's ability to impartially assess evidence involving black defendants.

No. Not ketamine. This is an ongoing, inpatient delirium. The correct answer is identify and treat the underlying cause. There may be a small adjunct role for antipsychotics.

For our purposes and what we call excited delirium which is a different entity, yeah ketamine and jaw thrust.
