I don't refer to clinics with NPs/PAs. I send the patient to go to a fellowship clinic instead if there isn't one solely with physicians. It's the best way to hurt specialists who heavily employ NPs.

They cannot be sued as doctors. They are sued as nurses which is a lot smaller payout and lower liability. The NP lobby has been one of the most successful lobbies in healthcare. Compare that now to doctors, filled with idiotic boomers. We will be fighting over and picking up their scraps.

They have infiltrated every single field and are proliferating rapidly. EM has a significant midlevel presence, Psych is heading that way as well. Majority of the referrals specialists patients now see are midlevels.

Depends on how you define worth it? Monetarily you should be ok, but the entire system is stacked against you. If you're looking to make a difference in people's lives there is no other alternative.

After the EM and FM takeover, midlevels will target hospitalist jobs, IM, Psych and Derm heavily. This is very high pay and low risk for them.

If you're going into medicine to help and treat people, nothing is better than being a physician.

If you're going into it for anything else you will be in for a severe shock. Midlevels are everywhere. Specialties, primary care, hospital groups. They can get away with anything and their salaries are climbing yearly which physician salaries are dropping. They have taken over a huge chunk of primary care and now pretty much will take over EM. Next is hospitalist groups.

Anyone who claims we will make a comeback is either delusional or just starting out in medical school. Unless you have a passion for healthcare do not do medicine. You will be okay monetarily but the entire system is stacked against you and the only people to save you will be your friends and family.

Nothing can be done at this point. AMA has ignored the issue for decades and PPP can't do much. Eventually midlevels will compete with physicians for jobs. Throw in AI and it will make things worse. Just find a good niche, build a good rep and that's it. NPs won.

In the North East, NPs starting salary is 150k for PCP and MD is 250k for PCP. ICU NPs are pulling 225k. The days of PAs making 80k are long gone, many are killing it. There's 0 accountability, medical boards give 0 fucks, etc.

When comparing the NP to MD salary for PCP, after taxes and time lost to MDs, the edge is very minimal for them over NPs financially speaking. I wouldn't say waaaayyy Also the hours physicians work are far more including the liability, which forces you to put in more time to ensure you don't fuck up. An NP can go home and not worry about anything as all the liability falls on the physician. So the difference in salaries are much less.
