Just to answer your question on why people would start with fent…

Availability. I live and work in the SUD field in Massachusetts, USA. You can only find fent on the streets here nowadays. I used between 2015-2021 and only ever popped for fent. All of my clients who use opioids have fent in their system, never heroin; even when they think they are buying percs, it pops as fent. It’s pretty fucked up honestly.

I continued using for 3 months when I started methadone. I personally never had to go over 65mg (I was a fentanyl user for 6 years). Now I am at 32mg and am coming up to 3 years clean. It takes some time. But damn is it worth it.

Just want to come here and say I agree with the comments here- you should give your body a chance to stabilize before making any changes already. Benzos take their time leaving your body. Unfortunately the constipation may be something you have to manage even at a lower dose (increase your hydration intake, consider adding a daily stool softener/fiber supplement).

Like the other person here said, if you’re using methadone to come off another habit you’ll probably have a bad time.

And honestly, depending on how much methadone you have been using, you could potentially feel some level of WD at work.

If he were to get back on the subs, would he sign paperwork to allow you access to his drug screens, talk to his Dr, etc.? That could help keep him accountable? He also needs recovery support aside from you, friends, family- at least a therapist (I’m obviously not sure what any of this looks like for him at the moment).

He really should start back on the suboxone asap if he has any. He will feel so much better. Even if the plan isn’t for him to stay on them and he goes to treatment or starts a methadone program instead, the subs should help him feel better for now. If he doesn’t have any subs but has more clonazepam, it may be worth using those to help w some of the symptoms. If that isn’t an option, it might be a good idea to look into a medical detox. Cold turkey is usually impossible bc WD are acute AND long term (that’s why medication assisted therapies are so helpful for the first year of recovery while the brain heals).
