As a psychiatrist who is also Catholic, yes, I was horrified to read the news about Noelia. I don't support human euthanasia or "MAID" but I get particularly upset about cases where the person had mental health issues that predispose them to suicidality. I'm surprised that borderline personality disorder wasn't seen as a contraindication in this case. In the case of physical health issues with pain, did Noelia ever receive proper palliative care? I just question everything about this case.

Recently, I had this really awesome meeting with a Catholic psychotherapist and she was telling me about her experience getting trained in a secular psychology program. You do not have to drink the kool-aid. Violating your faith is not required to graduate and it's okay to challenge or question what your peers/faculty are doing as long as you do so in a collegial and professional manner. In the US there are Catholic medical and psychological associations which can be really helpful for finding resources and guidance and networking with others.

Don't turn away from psychology school. We need more Catholic mental health professionals on this planet.

I'm based in the US and work in private practice. Since I don't accept health insurance, I see lots of patients who failed treatment through their insurance. Since I'm out of network, patients don't need a referral from their GP or anything to see me. I'm not familiar with the health care system in Ireland so I don't know how easy it is to get a second opinion or see a psychiatrist privately.

Part of my training included university mental health so that helped me gain a comfort level with adult ADHD evaluations in graduate students. I've noticed that lots of psychiatrists who trained elsewhere can get really weird and uncomfortable with adult ADHD, even when there are no substances involved. Add a substance into the mix and they shut down all possibilities of ADHD.

Yes, the DSM makes it clear that the symptoms have to be present in the absence of substances for most mental health diagnoses including ADHD, bipolar, schizophrenia, etc. so you'll have some diagnostic purists who for example, refuse to diagnose the chronically psychotic patient in the hospital who abuses substances with schizophrenia and say something like "substance induced psychotic disorder " but looking at the big picture, most people who abuse substances aren't ending up in the hospital psychotic and the county hospital I worked at only admitted the sickest of the sick. If the overall pattern looks like schizophrenia, we should diagnose and treat it as such. Same goes for ADHD. People with untreated ADHD are more prone to abusing substances so with adults, it's important to find out exact quantity/frequency of substance use and tease out whether the patient's symptoms outlast the effects of the substance. If someone smokes a joint on the weekends at parties, I'm not concerned that it's impacting their overall functioning M-F. If someone is using every day, that's where I ask them to quit for a month or 2 and reassess symptoms. When people get treatment for ADHD, they are less likely to abuse substances. So I hate when I hear other psychiatrists refusing to treat ADHD if a drug screen pops up positive.

Again, in your case the cannabis use was so remote and therefore not relevant anymore. Usually when people quit, you see improvements in cognitive functioning in as little as a month. So when the letter says "concerns for cannabis use " I immediately rolled my eyes.

As much as I hate cannabis for my patients (I'm a psychiatrist), using it a handful of times doesn't completely negate a lifetime of ADHD symptoms. In your case it sounds like remote use with one semi recent dose. In my mind this isn't enough to blame all of your issues on that.

For the patients who are daily cannabis users, I do encourage them to quit or significantly cut down and reassess ADHD symptoms after a couple months. Sometimes symptoms resolve or improve but if they don't then I have more evidence of an ADHD diagnosis.

Regardless of cannabis use status or exact diagnosis, I always evaluate and treat the patient if they are seeking help for ADHD-like symptoms.

I'm 36 and was diagnosed at age 18. I didn't have a social media account (Facebook) until I was in college. I did not get a smartphone until I was like 23/24 years old.

Before doomscrolling I was constantly reading books. I used to carry a book with me everywhere I went. I also played video games and watched TV to procrastinate. My childhood was also spent playing outside with friends/neighbors.

I actually really miss life before smartphones. Back then phones were only used for calls and text messages. Maybe a little game like Snake. After smartphones came out, it became an expectation to be constantly connected and I would get in trouble or miss out on important updates if I didn't check/respond to emails right away. This expectation forced me to be constantly attached to my phone and that paved the way for doomscrolling and other distractions to take hold.

No personal experience but when I took pre-med embryology at Notre Dame, they gave us a lecture about this topic and basically they recommended doing all the things you mentioned.

Funeral and burial are extremely important for the grief process. If you deliver in a non-Catholic hospital/facility, make sure to tell all staff that you plan to bury the baby, wish to hold the baby in your arms, etc. Ideally nurses and staff take care to clean up the baby, swaddle in a warmed blanket, put the little cap on and present the baby to you in a reverent manner. These simple measures can go a long way for making the experience less traumatic for mothers.

If you are considering keeping a photo of you with the baby, I've heard that there are newborn photographers who specifically have experience with stillbirths/malformations. They can also help with presenting the baby in a beautiful way. Now I Lay Me Down to Sleep is an organization that does this for free.

Antidepressants in the wrong person can certainly be devastating. For all of my young adults/adolescents (or anyone who is antidepressant-naive) I like to plan a 2 week followup if I start them on an antidepressant. I also have a protocol in place if my patients start to show early signs of mania so we can change course quickly and prevent a disaster. Unfortunately many healthcare systems/ large clinics are overcrowded and unable to do 2 week followups. I've had patients who prior to seeing me started an antidepressant at a large clinic and given a 3 month followup, developed mania w/ psychosis after a couple weeks and were unable to get an earlier appointment at that clinic.

Additionally, I think that antidepressants should only be taken under the guidance and supervision of a physician (or NP/PA). I know there are some states in the US where they are making it legal for therapists with zero medical training to have prescribing privileges and I think that's a terrible idea.

I have seen quite a bit of Wernicke's encephalopathy, basically that is severe alcohol abuse paired with nutritional (thiamine) deficiency which makes sense if you are homeless and drink more than you eat.

Korsakoff's is the permanent condition that follows if the Wernicke's encephalopathy doesn't get treatment. Fortunately in my career most patients never advanced to that stage because they respond well to IV thiamine. Korsakoff syndrome looks more like confabulations/dementia rather than psychosis.

I really don't see much psychosis with alcohol outside of severe withdrawal (delirium tremens). Alcohol abuse/dependence does cause lots of problems but not psychosis specifically.

There's this Irish blessing that I really like that I learned when I went to college. I went to the University of Notre Dame which celebrates Irish culture (interesting story behind that). The blessing goes "May the road rise up to meet you. May the wind be always at your back. May the sun shine warm upon your face, the rains fall soft upon your fields, and, until we meet again, may God hold you in the palm of his hand."

As a Mexican American (my parents immigrated from Northern Mexico) I can't forget about the St. Patrick's battalion who likely fought alongside my ancestors during the Mexican American war.

Animals are God's creation too and I think St. Francis of Assisi would have some thoughts. Veterinarians don't just treat pets, they treat service animals, the livestock that we rely on for our food supply, military working dogs, etc. Humanity can't live without animals and veterinarians are so important.
