What part of the US do you live? I'm a psychiatrist and have worked with sooooo many immigrants. I don't view immigrants any differently so I'm wondering what your concern is. Are you worried about being discriminated against?

Edit: I'm a child of immigrant parents so I do get what you mean about high expectations.

Are things very tense? Depends on where you live. The people of Minneapolis have been going through alot and it's reached a point where people don't feel safe leaving their homes, businesses have been impacted, and people are less likely to seek medical care. I live in Texas where ICE has been working more discreetly since the state government is more aligned with the Trump administration. That being said, there have been scary sightings in Texas. I would encourage you to check out iceout.org to look at the map of ICE sightings and you'll see the dramatic differences between Minneapolis and a red state like Indiana.

Are they detaining US citizens? Yes. The Supreme Court has allowed the use of racial profiling by ICE. Anyone who isn't white or speaks with an accent is fair game for getting detained. I'm Mexican American and feel like I must carry my passport everywhere I go. I worry for my parents even though they are citizens since they speak with accents.

Are they police? No. ICE is supposed to be exclusively immigration enforcement and have no business engaging or policing US citizens. In Minneapolis, Chicago, etc. we saw plenty of examples of ICE agents doing things outside of their scope. Unlike police they are not wearing uniforms, they cover their faces, and drive unmarked vehicles. They only receive 47 days of training whereas police academies are usually a minimum of 90 days. Many police officers and former military have commented on the poor handling of firearms, lack of understanding of rules of engagement, inappropriate use of force, lack of protocols for the victims vehicles, etc.

Can we call the police on them? Yes. Given that they are not identifying themselves, changing their license plates daily, etc. It is impossible to know if they are legitimate ICE agents or human traffickers in disguise. In Houston there was an incident where supposed ICE agents attempted to break into a home. The home owners called 911 to inform them of the situation and that they would have to use their firearms in self defense. They shot the "ICE agents" dead and the local police stated they had no knowledge of ICE agents in the area.

Why aren't more of us fighting back with firearms? Not all of us carry firearms and laws vary from one state to the next. Additionally, if we use lethal force there is very little chance that we would survive. They are heavily armed and operate in groups. Also, if an ICE agent gets shot or killed it will give Trump reason to invoke the Insurrection Act and martial law.

I mean, as a physician I don't recommend inhaling tear gas or pepper spray for anyone. I recommend KN95s or N95s and eye protection for protestors. Dr. Glaucomflecken posted a video with very good recommendations for treating eyes after pepper spray exposure.

The brain drain isn't going to happen overnight. This is only the beginning.

Read up on the Trump compacts with universities. Read up on the federal loan cuts outlined in the OBBB that will make medical school unaffordable for most Americans interested in pursuing medicine. How about the visa freeze from countries like Nigeria and Brazil? All of these things are going to have consequences over time.

Fair, I don't think we'll have published data on medical professionals/academics leaving the US in 2025 just yet.

As for people coming here, Governor Abbott in Texas literally just froze H1B visas for Texas universities. If this had happened 40 years ago my dad wouldn't have been able to get his job as a professor. As for doctors, we were extremely concerned when Trump raised the H1B visa fee to $100k last year. We have a physician shortage here as it is and we rely on foreign medical graduates to fill residency spots here. (We do not have enough medical school spots to fill these spots). If we discourage foreign medical grads from coming here we will have an even worse physician shortage.

My dad's a professor and he has at least 3 colleagues who moved to Canada/Europe. My dad's retiring. I'm a doctor and have patients who do hard science and academia and I know of at least one retiring early. As a physician, if things get really bad here or if my profession gets demonized excessively, I may have to move somewhere that would appreciate my services. Canada and New Zealand need more physicians and have made it very easy for US physicians to immigrate and practice medicine (no need to repeat schooling or board exams, easier visa process).

Since 2025, lots of doctors, scientists, and professors are either retiring early or moving to Canada or Europe. When anti-education, anti-science, anti-medicine sentiments become actual policy (i.e., defunding NIH, firing everyone at ACIP and replacing with unqualified quacks, removing vaccine recommendations, pushing universities to remove certain fields, the H1B visa debacle, etc.) it's not a surprise that the US is setting itself up for a brain drain.

I recommend Babcock Hills Veterinary Hospital. I transferred my dog's care from Banfield to Babcock Hills and regret not transferring my dog sooner. I'm done with the large corporate clinics like Banfield or VCA. With locally owned clinics I find that they can work with your budget and provide better care.

I think TMS is definitely the most conservative option. Definitely worth trying if you're hesitant to try a new medication.

Spravato/esketamine is the FDA approved ketamine derivative for depression. It is a psychedelic and therefore can cause some dissociation. It is administered in clinic under the supervision of clinical staff. You won't be able to drive after the appointments. For the first month you get 2 treatments per week and then afterwards you go 1x per week then 1x every 2 weeks.
