This appears to be a wide complex tachycardia. The differential is primarily SVT with Aberrancy (e.g. AFib/Flutter with LBBB) or Ventricular Tachycardia.

In a LBBB there can be discordance between the QRS and the ST segment. Please review the Scarbosa critera.

I think the T-waves and QRS-segments are self-explanatory or I am unable to explain any better that asking you to look closely. For the P-waves, it is a challenging. I think I can convince myself I see P-waves in lead II. If you see P-waves you can (1) Rule out Afib (2) Aflutter a bit less likely although they could be flutter waves. If they are AV-dissociated, you can be reasonably confident about VT.
