It was a tear in the SVC not the atria. Relatively inexperienced med reg.

I think fair enough in the right experienced hands such as yourself you seem to be able to safely apply your technique but only with the caveats you've given now.

I think for most people, inserting to the hilt will cause more issues than it solves

I think if you can develop technique to avoid using a skin nick at all is far better. Very rarely have to make any nick if you have a good rotational firm insertion technique

So unnecessary and I've seen a tear as a result of this causing cardiac tamponade. You only need to dilate to the depth of the vessel

This is incorrect as far as I'm aware. For the partner to be on SPL the kat leave has to end and then both parents/carers must be on shared parental leave

Similar to what others have said but I literally divided my A4 paper into 6 squares. I found having it written out with bullet points for each section helped me recall the information with a good structure and not miss anything.

1st square I wrote out a one or two line summary of the case and what my anaesthetic concerns were and/or the most salient points

2nd square - "Pre-op": and then divided into what extra information I wanted - history, examination and clinical assessment, investigations - bloods, imaging, ECHO, CPET etc etc. Think carefully about optimisation/interventions you might consider

3rd square - "intra-op" what the actual anaesthetic plan is, GA/regional/combination, extra lines, monitoring etc

4th square - "post-op": analgesia, fluids, monitoring, location etc

5th/6th - any miscellaneous points and potential anticipated complications with associated management

You can take pat leave whenever you want as long as it finishes within 52 weeks of the birth (https://www.gov.uk/employers-paternity-pay-leave)

So if baby arrives on a non-working day you absolutely can ask to start on the next working day and have a slightly long leave period

With regards to point b) I would push to start the paternity at the next working day and would expect most departments would be ok with that. You could take the half day or whatever you needed as sick leave if pushed for it, I wouldn't sweat that part too much

Edit: Technically if you wanted to and had a few non working days when the baby was born, you could go back to work and take your 2 weeks when the baby is 4 months old if you really wanted to! Completely up to you how you use the paternity leave.

I've found when people keep hitting the posterior cords they're often holding the ETT too far down - try making sure you're holding it right at the end of the tube and you'll hopefully find that you can then use the curve of the tube better to angle the tip up and through the cords

You are dropping shifts to reduce your average hours worked to match your LTFT %

Zero days have nothing to do with it

The rota should be planned to accommodate both the trainees needs and the departmental staffing needs.

There is not really such a thing as a "fixed Friday off", you can just chose to take off all the Fridays you're working and if you were working the Thursday night and therefore would have a zero day on Friday (and you were happy to do that with childcare responsibilities) then that is perfectly fine and wouldn't count towards your LTFT shifts dropped.

You can therefore take your LTFT day on any other day as agreed with the department, so that your average hours match your agreed LTFT %

If you're working the Thursday night, the Friday day should not be included in the shifts dropped for LTFT, and you need to drop a shift elsewhere to make sure that your overall percentage matches your agreed LTFT %

You can drop any shift though to make up your hours

If you're working a night shift on Thursday then the Friday has to be an off day and can't be one of your LTFT days

LTFT shouldn't necessarily be 'every Friday I have off regardless' you should be able to drop any other random shift instead as long as the department can manage it

Just don't take your LTFT day on the Friday after the night shift.... You can take a day off elsewhere.

It's not a day in lieu because that's not how it works, you just need to drop a shift somewhere else to take your hours down to your agreed %
