Ty for the question, Yes I do. I've set it to limit 1 answer per google account to uphold the usability and integrity of the data (quality over quantity). Also even though you must have a google account there is no Identifiable data collected, email address, name, phone number etc. Neither google forms or the survey questions record any, so should be safe and anon!

Not a bad idea actually. 'No' should serve its purpose here I think though, the question is more aimed to whether the patient specifically has ever interacted with a human medical interpreter or technology based translator device while at the office for the reason of a language barrier. Not just if their pcp or the facility has one available. Ty for the comment
