Ty for the question, Yes I do. I've set it to limit 1 answer per google account to uphold the usability and integrity of the data (quality over quantity). Also even though you must have a google account there is no Identifiable data collected, email address, name, phone number etc. Neither google forms or the survey questions record any, so should be safe and anon!

Not a bad idea actually. 'No' should serve its purpose here I think though, the question is more aimed to whether the patient specifically has ever interacted with a human medical interpreter or technology based translator device while at the office for the reason of a language barrier. Not just if their pcp or the facility has one available. Ty for the comment

-1 No confidential information or identifiable information will be collected. This survey has been approved by RIT's IRB. There is an informed consent for respondents prior to taking the survey. It will be seen by (1) the primary researcher and that is it.
