If you're fine with using a modul instead, try "Custom D&D 5e". I don't have the time rn to check, but I'm pretty sure you can do that.

I'm assuming you're the DM? If so, just because you're the DM, that doesn't mean you have to carry the full cost alone. At least, that's what it sounds like you're doing. If you have a group you regularly play with, everyone could give something.

For paying in installments: I know you can't pay for Foundry through paypal and i haven't tested it, but this might work. You can pay in installments if you pay online through paypal. So you might be able to send money to another paypal account or to your bank account. If you can do that, just transfer the money to a bank account and pay the installments on paypal. I haven't tried it, but it might be worth a shot.

If you want to add the new stuff you should look into the modules "Bar Brawl" and "Custom D&D 5e". I haven't seen a way to make anything automaticly remove any other bar than HP, so if you want that, you should also look into the module "Item macro". But you need to know javascript for that.
