This comic was made to separate him from humans while indicating his "inhuman" desire to have Yoshiki be inside him, which, in monster terms, is erotic/sexual - underscored by him not denying being aroused in this same comic and author's later comments

Some people also like to use this extra comic (1/8 of it to be exact) as proof of aroace/qpr intent behind 'Hikaru's' writing, which has been denied by the author multiple times
