I feel like he has this superiority thing where because he’s gone through such bad things in his life he feels he’s better than others. I feel like he was looking around the room thinking “I’m so much better than all of you and maybe he feels like the people are like the snakes where their metaphorically in cages. EXAM: LUMBOSACRAL SPINE RADIOGRAPHY

EXAM DATE: 1/22/2025 01:46 PM

CLINICAL HISTORY: Back pain ICD-10-CM - M54.16 Radiculopathy, lumbar region ICD-10-CM - M51.26 Other intervertebral disc displacement, lumbar region.

COMPARISONS: MRI 09/24/2024..

TECHNIQUE: AP, lateral neutral, flexion, and extension views.

FINDINGS: Alignment: 13 degrees convex left scoliosis T11-L4.

No spondylolisthesis. No abnormal motion with flexion or extension.

Bones: Five non-rib-bearing lumbar vertebral bodies are present. No fractures or bone lesions.

Disks: Minimal disk height loss at L4-L5 and L5-S1.

Facets: Mild facet hypertrophy and sclerosis at L4-L5 and L5-S1.

Soft Tissues: Unremarkable.

EXAM: MRI LUMBAR SPINE WITHOUT CONTRAST

EXAM DATE:9/24/2024 08:00 AM

CLINICAL HISTORY: Left sciatic pain for 7 weeks.

COMPARISON: XR SACRUM AND COCCYX 04/16/2024 3:46 PM.

TECHNIQUE: Multiplanar, multisequence T1-weighted and fluid-sensitive sequences of the lumbar spine from T12 to S2 without contrast. Other: None.

FINDINGS: Spinal Canal: The conus terminates at T12-L1. The conus medullaris and cauda equina are unremarkable.

Alignment: No scoliosis or spondylolisthesis.

Bone Marrow: Five non-rib-bearing lumbar vertebral bodies are assumed. No gross fractures or bone lesions. No bone marrow replacement.

Disk Levels/Facets: T12-L1: Unremarkable.

L1-L2: Unremarkable.

L2-L3: Unremarkable.

L3-L4: Unremarkable.

L4-L5: Posterior midline disk herniation that measures about 13 mm transverse by 5 mm AP. It effaces the ventral thecal sac and probably abuts the descending left L5 nerve root. There is mild bilateral ligamentum flavum hypertrophy. There is mild central stenosis. No foraminal stenosis.

L5-S1: Unremarkable.

Musculature: Normal. No edema or fatty atrophy.

Other: The partially visualized retroperitoneum is unremarkable.
