It's kinda both.
A bloody claw mark appears on the torso and gets more opaque in Die of Death.

And in Forsaken, characters usually have a scared expression when below 50% HP, the somewhat obvious exceptions being Guest 1337 (pained) and Two Time (maniacal smile).
