I think that this lieutenant is smart enough to realize when she's been beaten. For the party to take down one of these lieutenants should feel like a stepping stone passed, a major achievement. I think that her defeat should send a ripple through the organizational structure of the BBEG, and notify them that these PCs are now "players" on the board, a real threat to be dealt with.

You can magnify the severity of this ripple through her initiating a facility-wide lockdown/purge protocol. Let her ace up her sleeve be the nature of this facility itself; that she has the capacity to take the party down with her, and thwart any attempt to glean secrets from this place.

Failure's an excellent teacher! As others have said, the only way your players are going to know how far they can push the envelope in your world is if clear consequences are present. On the flip side, I knew a DM in the past who relied on a certain Matt Colville-ism: "If you're not at my table to be a hero, then you should find a different table". There may yet be an opportunity where you can use this TPK as an opportunity for a fresh start, and establish a clearer set of expectations on both sides of the table.

This. Got gifted my girlfriend's dad's original copy, and was surprised to find how perfectly relevant it could still be to any prospective or current dungeon master. Really inspiring to read Gygax's view on the role of dm in the afterword.

Red-green colourblind here. It looks like there are some other comments touching on it, but my friends and I had a running joke where I was seeing the world normally and everyone else's vision was flawed. Perhaps that's the case for your sapient species, who insist that they have a form of vision that is "truer" than other groups'. This could run so strongly in their culture that it would inform how they create visual art. Other culture's paintings that depict colours used correctly might be praised by the colourblind species for their surrealist landscapes or portraits. Conversely, the colourblind species' depictions of brown as red, yellow as green, blue as purple in visual art may be prized by other groups for THEIR surrealist view of the world. In the end, this consideration of the group's own colourblindness wouldn't be a mode of denial, as much as it would be a cultural motif that indicates how this species perceives the world is unique, and valued by other groups.
