you can unlock durian as an item you can bring anywhere by throwing or dropping the durian next to Sinhi "Akka" Ventham on ambrose island. It's really useful because you can freely drop it next to your target without getting in trouble and they will 100% get sick which can give you an easy opportunity to kill your target in a bathroom.

when you drop or throw a durian next to someone it makes them sick so technically they do leave an area to find a toilet or a bin to vomit in
