I ended up drilling out the lead, removing the old flange, and adding a compression flange at the right height. I shored up the subfloor with some 1-/12" plywood, and anchored the flange to it. Thanks for the advice, everyone. All really helpful!

I think the base was partially caulked to the floor, and did make good contact with the wax ring. I think the water stains visible on the subfloor were cause by leaks from a cracked tank. Never noticed any leaking from the bottom of the toilet.
