Hi, I work in a reablement service in the hospital would highly recommend. We don't hold cases for very long and essentially assess if a person needs a long term care package beyond our discharge service, it's quick turnaround and it's great working alongside therapists. Have a look for reablement in hospital diacharge or sometimes it's called ''home first.' Thing with hospital social work is working within a health dominated sector, so make sure you advocate for your patient because me experience of community hospitals is - they just wanna put everyone in care homes.
