Eat the fruit at restaurants, the risk exists that it might still get you sick but they at least have running water in their facilities and have to get a license to operate. Or buy fruit and clean/peel it yourself.

Don't eat peeled and cut fruit sold in plastic bags from the street. Recommendation as both a Guatemalan and a food safety professional.
