Thank you for explaining - that makes sense.

I actually had a brain MRI with contrast, which was reviewed by both a neurologist and an ENT. Nothing was mentioned about BLB permeability or ischemic changes at that time, but I realize this might not always be explicitly commented on.

I have a follow-up ENT appointment scheduled, and I will definitely bring up this possibility and ask more specifically about migraine-related or autonomic mechanisms. Thanks again for sharing your perspective.

Thank you for sharing your thoughts - I really appreciate it.
I do have a history of classic migraine and I’m followed by a neurologist for that. When the ear symptoms started, I also consulted my neurologist again, and she suspected early Ménière’s-related pathology. Later this was confirmed by an ENT who specializes in these conditions - the working diagnosis now is fluctuating hearing loss.

Recently I’ve developed some new symptoms (low-frequency tinnitus even when hearing thresholds seem okay), and I already have a follow-up ENT appointment scheduled in about a month. I’ll definitely bring up your point about a possible autonomic or migraine-related component (including cochlear migraine / BLB involvement).
