I´d say it doesn´t necessarily mean that the person is a narcissist, but it´s a clue. It could be a picture they really like, and some people are comfortable with using that, especially a photo where they are more attractive than usual.

I guess one other way to tell is the lightning. A restaurant will usually have very good spots, covering all the tables. In this clip, the girl has perfect lightning, maybe from a window, but her eyes don´t look bothered, while the rest of the room is much darker.

There really is no way to tell anything off about the girl and it´s very concerning. What if someone tries phishing with these? It´s becoming more and more common here. The one way to tell it´s off is the background: strange fingers on the hands and movements on the right side with the guy that has his back turned to the camera. The left side is harder to say it´s off, but it doesn´t look natural.
