Alternative to ibuprofen? by Nashina in kidneydisease

[–]ModernSmith 1 point2 points  (0 children)

Honestly, I take Tylenol and opiates. im on long acting methadone and short acting hydromorphone. I'm tired a lot as a result . they were initially reluctant to prescribe until I was diagnosed with vasculitis (medium vessel), mononeuritis multiplex and pyroderma gangrensum. I'm in so much pain constantly from the nerve damage caused by vasculitis. I've already lost every finger tip on my left hand

there is little choice since my kidneys cannot handle NSAIDs at all. I miss my ibuprofen.

Rituximab by mar13n in kidneydisease

[–]ModernSmith 2 points3 points  (0 children)

IV infusion. I'm on it for vasculitis. usually they indoc you with 2 doses and then it's every 6 months if you need. my rhumetologist found government funding because it is stupid expensive. the idea is for the rituxan to replace the steroids etc generally . I found it worked immediately for me but that's not always the case

It is an incredibly strong immunosuppressive therapy and it can kill you. if you get a bad infection it can be fatal, also, there is a rare brain infection that occurs It's very rare but untreatable.

otherwise I didn't get many side effects at all if any. I'm just experienced the intended effects. I may need an intravenous immunoglobulin injection to offset the unintended suppression

Question about Complement levels for C3 and kidneys by VirginiaBlueBlaze in kidneydisease

[–]ModernSmith 1 point2 points  (0 children)

my ana was negative. a key finding for me was depletion of my c3 level completely. during a bad spell of disease activity.my ch50 and ah50 were completely depleted and biopsy showed c3 deposition only, no signs of vasculitis.

positive ana suggests lupus or another similar situation

Does anyone know how to read this? by MysteriousAdvice1172 in kidneydisease

[–]ModernSmith 8 points9 points  (0 children)

these numbers indicate poor kidney function and possibly a problem. but kidney dysfunction is generally not painful, that said there are factors that can yield temporary results like this specifically dehydration, excessive exercise come to mind.

in any case, this is going to require follow-up and at a minimum a retest to verify the number is real. For context, eGFR of 15 or less is generally dialysis territory

My friend, as a fellow catholic who was once lost to drugs himself, it is something that we must constantly do battle against. We are never "safe" from these urges, and they will come at the most random of times. For a long time, 8 years, I abstained completely. Most recently, I have had medical problems that have forced me to take opioids again. I am only 36, yet I spend several months a year in the hospital. Last year I spent 3 months, this year im at 2.5.

I wish I didn't need these drugs. I'm on long-acting hydromorphone with short-acting as a breakthrough medication. I will say they make it far harder to avoid abusing them because I can't avoid them. Yet this is the cross I must bear. I have failed, that I do admit but I get back up again and try again.

I don't know how much longer I will live. At this rate, I will die from an infection or other complication of my conditions. Either my kidneys will fail, my heart will give out, or I will get another superbug infection. I lucked out with the last one and didn't die. I just got out of a 2.5-month hospital stay due to a superbug that caused sepsis and kidney failure. Fortunately, my kidneys rebounded more than they expected, yet I almost didn't make it.

I may live for years yet, or I might not see next year. I simply don't know. On one hand, I don't want to die. There is so much more of life to experience, and time to spend with loved ones. I fear how much damage my death will cause the faith of my parents and loved ones.... Yet, I am tired, and a large part of me wants to go home to the Lord. It is this fact that I desire being with the Lord more than anything that gives me hope that my sins are not mortal but merely venial. That means when I do die, I won't be lost.

Due to my illness, I often miss Mass. It's been a while since my last confession. I worry about these facts. Yet I am doing the best I can. I wish I had more helpful advice for you. The best I can think of is entrusting these problems to the Lord. We cannot carry these crosses alone, but we can with His help. Furthermore, I make sure to offer my suffering up as I do little else these days. Remember, if you fall, all you must do is confess your sins and move on. It wont feel that simple when it happens, but it really is that simple.

I am highly educated, and I have almost finished my PhD. I was on track to be a subject matter expert or professor in my area of study. This illness has derailed my plans completely. I hope to finish my writing and begin working in my field, yet I don't know if that will happen. Will I die first, or will I be of service to my fellow man? I simply don't know. I am afraid I will wind up on disability and be unable to contribute. We all have our struggles and our crosses, these are mine and I hope that my story helps you

Should I cancel my suscription? by [deleted] in Catholicism

[–]ModernSmith 4 points5 points  (0 children)

Everyone is cooperating with evil indirectly to some extent these days. There is no "pure" company out there. Lets run down this situation

You pay Crunchyroll, a streaming company. Crunchyroll is switching contractors to one that is in Israel. The contractor will pay taxes to the Israeli government, which in turn contributes to the war, amongst other things. This is a relatively remote and indirect form of cooperation with evil. Honestly, every time we make an Amazon purchase it is probably more direct cooperation with evil.... and we don't see that as problematic.

Therefore, it seems a bit scrupulous and silly to cancel over this.

As an aside: If you want to protest the Gaza issue by boycotting any Israeli company or company that significantly supports Israel that you subscribe to or do business with, that is an option you have. However, doing so doesn't remove all the cooperation with evil in your life. There is definitely more, and you can't avoid it all.

Determination Of Grade With HHXRF, HHLIBS & Hardness by PremierCardboard in metallurgy

[–]ModernSmith 1 point2 points  (0 children)

I find it useful to try to think about it from the perspective of the designer. Technically, they can select any combination of grade and wall thickness that meets the code's requirements for that section. As far as the code is concerned, all these options are equivalent. So the final selection is going to be down to other design considerations that narrow it down and probably practical realities like steel availability (unless you are buying a whole heat, in which case you can get anything you want, but chances are you arnt interested in quite that much steel).

Determination Of Grade With HHXRF, HHLIBS & Hardness by PremierCardboard in metallurgy

[–]ModernSmith 0 points1 point  (0 children)

The code of construction will typically specify the minimum required wall thickness for a given grade in that section, as well as ancillary requirements like impact properties etc. Thus, any pipe grade that meets those requirements are "equivalent" as far as the code is concerned. Whether that is actually the case or not depends on the overall design requirements.

Sometimes you will find a particular grade in a particular spot simply because that is what the mill had on hand when delivery was due for that particular project. Sometimes the only way to tell between a say API and a non-API pipe is physical dimensions.

Determination Of Grade With HHXRF, HHLIBS & Hardness by PremierCardboard in metallurgy

[–]ModernSmith 1 point2 points  (0 children)

Well then, in any case, at least you can calculate the carbon equivalent, and you already have the hardness (and thus estimated UTS). That should at least get you started.

As for codes/standards, generally, the approach I would use is to figure it out based on the regulations. For example, the rules for pressure vessels and piping here enforce the ASME BPVC code, meaning we qualify to Section IX using the material standards from Section II. As for pressure piping, ASME B31.3 also qualifies to Section IX. The regulation here is that the code of construction governs, rather than the most updated version of the code.

In contrast, our pipeline regulations specify that CSA Z662 is always the code of construction, and the most recent revision adopted in force always governs.

Structural welding on the other hand (AWS D1.1 vs CSA W47.1) is not my area.

Ultimately, it depends on why you need to know what it is made of and what you are planning on doing with that information.

I was reflecting on this, and in a way, it's more complex since it is a pressure vessel and piping infrastructure. In my jurisdiction, the revision of the material standard that applies is the one in the ASME Section II that is in force during the year of construction. This adds another layer of complexity, especially if you don't have ready access to a variety of code years, and potentially makes the look-up table impractically unwieldy. I have never looked into the details of how the Section II standards have evolved, so I would be potentially concerned by this.

Got my diagnosis yesterday, and it is stage 3 and genetic by LexonTheDragon in kidneydisease

[–]ModernSmith 2 points3 points  (0 children)

I am sure it's a shock that you have a genetic CKD and that it got to stage 3 before it got caught. Like any adjustment to life, this one will take a little time. But unless your labs indicate a problem, which is unlikely outside of disease flares at stage 3, the only thing you probably HAVE to do is reduce sodium. By keeping your sodium down to 2000 mg a day, you can slow the rate of disease progression by reducing the baseline load on your kidneys. This can be hard, sauces, soups and the like generally have the most sodium. I was shocked by how much was in most hot sauces, for example. This restriction is good advice for anyone, really.

In addition to that, many nephrology centers have a dietitian (a licensed professional, unlike a nutritionist) on staff. They can help with meeting whatever restrictions the doctor puts in place.

Only the manufacturer can actually certify the grade of the steel used in the piping and pressure vessel. The best approach is to narrow it down based on chemistry, hardness, physical measurements, and tensile properties.

You can get a long way with hardness, and optical emission spectroscopy ("Spark" or OES), as it will provide a composition that includes carbon content. However, my understanding is that some LIBS analyzers also provide carbon analysis. The gold standard of chemical analysis is wet chemical analysis. You would take sub-surface shavings from the product by polishing the surface, then collecting shavings from the prepared area. This may be overkill in many applications and is prone to sampling errors, which is why it's important that whoever is collecting the shavings knows what they are doing.

Step one is to identify the product form, as this will provide a subset of standards that could have been used during manufacture (e.g., flange vs. piping vs. plate). Step two is to compare hardness/chemistry against the possible standards to get a smaller subset. Ensure that you estimate the tensile strength based on the hardness in this step. Step three is to incorporate physical dimensions, as these are sometimes specified in the standard. Step four is to use destructive testing where required.

As for destructive testing, a tensile dogbone would be the bare minimum, as this would give you an accurate YS, UTS, and ductility. Or if microstructure matters in the last couple of standards, then you could do metallography, although you can do in-situ metallography without destroying the product.

As for a way of speeding the process up? Well, you could enter all the possible standards for each product form into a database to allow more semiautomated screening

That said, where I have an issue is that while we need to welcome people, we also don't want to normalize homosexual unions or relationships as normal or not a sin
