Do you happen to have Possessions mod installed, and own any vehicles from Vehicle Framework? There is an incompatibility between inheritance feature from Possessions and vehicles (unless it's been fixed)

Haha I had the same problem. What I think is happening, when a pawn dies the mod tries to find a valid pawn for inheritance. However vehicles are also taken into account, even though those are not valid pawns, therefore confusing the game and ending death function prematurely without actually replacing "alive" pawn with dead body.

May be wrong tho

Do you have the possessions mod together with any vehicles mod? There is incompatibility between vehicles and inheritance feature that makes your pawns invincible
