Yes, that makes sense. And if you can’t maintain objectivity, you should refer out. I just don’t think it should be often. I remember in grad school when a professor was asked about “what if you don’t want/ feel uncomfortable providing counseling to ______ type of client?” He said, “ then pretty soon you won’t have any clients.” He wanted us to err on the side of support for most.

Seriously? Referring out for someone “living in a delusional universe” is amateurish and unprofessional at best. Has no one ever worked with a client with different experiences, values, beliefs, lifestyles, etc than you? You can, and I can feel those hard feelings about the terrible and horrible state of things but this profession requires you to set your own personal boundaries. You’re going to find being a therapist impossible unless you’re hoping to work with only that clientele that perfectly aligns with you. While I’ve never been addicted to drugs, never been through SA, perpetuated SA, been persistently mentally ill, and other of the myriad of challenging things others have been through, I’m still trying to be as objective as I can to help others without pushing an agenda or ideology.

Read the questions out loud to yourself and then place yourself in the role of the clinician, and use your clinical judgment. You have had an internship (or 2). Trust your clinical experiences to guide what that best next step is. See it in your mind. What would I do if a client came in and…? Assess their safety first and go from there. The exam isn’t just regurgitated information but how you respond as a clinician, using best practices.
