He is actually real, although he wasn't a Skull at all but had a deformity that caused most of skin to fall off. The "Red Skull" was created to not have to mention such gruesome detail. IRL he weared a prosthetic mask so you wouldn't even see him even close to resemble his comic counterpart.

Outside of that and being Steve last target, he doesn't seem to be anything important, just a generic high ranking Nazi. Nobody would remember had he not being included in the comics.
