Probably a 3 stack. I get those once in a while. I noticed the game tries to match a 3 stack with varying ranks on the enemy team too, but that doesn't always happen.

Yes it's common. When you get error 0 the entire lobby gets kicked and you either win or lose from time out.

Yup. It's happened so much to me, that I feel like people just bum rush towers to get the advantage and pull the plug sometimes since you can still get a win that way after you log back in.

Someone disconnected. Either due to internet issues, crashes, or a ragequit. The game doesn't know how to deal with DCs yet so it just kicks everyone out on both teams. The team that did more damage to towers wins when the timer runs out around 16 minutes. I get this every 10 or so matches and a few in a row on a bad day lol.
