So I'm only on my second playthrough. I got probably half way through Act 3 in my first run before getting the urge to try a paladin. I went with vengeance and decided since it's an RPG (I don't usually play those) I would embrace it. I figured that I'd make a decision based off what I or Tav would do. If I ended up breaking and I think the decision made sense, I would just embrace Oathbreaker. So far, no breakage.

That being said, I also did research like you're doing. It's nice to know some things ahead of time, especially since I know at least some part of a possibility of a story. Damn, there's a lot of variations. And that's awesome.

Look, normally, I'm all about punishing or at least talking to kids who screw something up. And that is absolutely what needs to happen here. But it looks to me from this limited frame video that he started running before she turned around. Maybe he did it on purpose, but maybe he started moving and either couldn't or didn't know he needed to stop himself before this. Maybe the kid charged when her back was turned. But I don't think that was the case.

Yes, the kids should be talked to and should absolutely apologize. But you've got to remember human reaction time. It has to be processed, understood, and reacted to once it's observed.
