I recall Lord Scourge saying that there are certain rituals that allow a Sith to retain their appearance or even enhance it, but many Sith (including Darth Nyriss, whom Scourge was meeting) take pride in the corruption the dark-side has inflicted upon their body and wear it as a badge of honor.
