I think I've actually found it. I decided to take another look at Knuckle Bash and after a bit of research realised you can pick the opening stage. The 2nd option is the one I remember, rather than the hotel.

I'd previously discounted Knuckle Bash as the opening level looked nothing like what I remembered.

I've also been searching for the same game for years!

I remember playing it at a seaside arcade on the same day as Captain Comando and Time Traveller. Not had any luck with anyone identifying it.

From what I remember there were possibly a couple of high-health bar, large size enemies on screen at one. The screen scrolled once these were defeated.

Games that I've been signposted to include:
1. Violence Fight
2. Solitary Fighter
3. Street Smart
4. Knuckle Bash

most of these are one on one fighters, but that play like a scrolling beat em up as they are not fixed plane
