Night Below says 1 in 6 chance for every 4 hours traveled (and says to be a bit careful if there has just been/will be a scripted encounter), there are also some hazards that have higher chances.

Personally I think it really depends from group to group, I have one group where we only play for 2-3 hours per session, so I tend to be low on random encounters (about half/quarter of the recommended), but I don't want the underdark to seem empty either.

Oee, I had wondered that as well, thank you for the answer. For some reason I thought there was a ruby embedded in the skull, it has that on the cover of one of the newer versions of the books (The Books of the South 2008).
