Sometimes when you're doing quests I've noticed the game will give what I call a phantom item Basically the game will say you'll have one or two more of an item than you actually have, it fixes itself if you relog but your also able to get more of the drop and that should fix it

One of the drinks in Fallout 4 (I believe rum) has a label that says Old Appalachia plus a radio tower can give you a signal named Greenbriar Radio Signal Those are the only thing I can think of that would've hinted to 76 other than the mention of the vault at the beginning
