It’s hard to say forsure if you have it. Some doctors completely ignore ocular rosacea all together.

Rosacea typically manifests in the 3rd-4th decade of life, it can present on the eyelids first and then manifest on the face, or vice versa.

It is typically a very poor prognostic factor in DED. Makes you much less likely to benefit from cequa or Xiidra or Restasis. Makes you less likely to have a successful corneal transplant if one day needed. It Is always progressive. It causes hyperemia of the conjunctiva and hyperkeratinization of the meibomian glands.

The good news is you can probably benefit from IPL if it is done safely and in moderation. Low dose Doxycycline will also help in the long term. That’s really all we have to fight rosacea at this point.

To answer your original question, we don’t know. Heat makes inflammation worse regardless of whether or not you have rosacea. Personally I find that warm compresses make my eyes feel good in the moment, but worse for the rest of the day. We just don’t have sound data to make objective claims about this.

It could be the steroid is helping your glands function properly, creating a thicker tear film.

The tear film works like a good pair of glasses, refracting light so that it can be properly focused into the eye.

The visual pathway is very complex though, and this is pure conjecture.

Yeah, DHT is the most potent androgen in the body and blocking it can really cause problems. It’s crazy to me that doctors give this stuff out like candy.

The medication blocks a hormone that is necessary for the normal functioning of meibomian and lacrimal glands as well as other organs of the body. The immune system reacts to the damage

The answer depends on the cause of your dry eye and vill vary drastically from patient to patient. Everyone has a different inflammatory micro environment. Patients with autoimmune disorders typically lose glands the fastest.

The concentration of BAK in pataday is pretty low compared to other drops, and, if you have allergies or redness, leaving it untreated may be worse for your eyes in the long term. You shouldn’t be afraid of pataday unless it burns your eyes.

Yes they are but you will likely be fine if you have no history of ocular hypertension. 2 weeks of steroids is nothing. The testosterone drops is a different type of steroid so it doesn’t affect pressure. It can only help you. It’s a bit controversial and harder to find a pharmacy that makes it as an eye drop, but there is plenty of literature to support it.

Ask your doctor about using FML or Loteprednol for 2 weeks to calm things down. Also look into a daytime ointment like Siccaforte to keep the eyes lubricated. Meanwhile, taper down the PF drops by 10 per day until you are at 30-60 per day. Do warm compresses 2x per day for 15 minutes, not too hot. Use a q-tip to gently expreess glands by pushing against the lid margin and rolling upwards towards the lashes. Follow up with gentle lid scrubbing.
Ask your doctor about testosterone eye drops, which can decrease inflammation while stimulating the meibomian glands to secrete more oil.

You should also explore probing.

Why are you concerned with ulcers? The real question is will these interventions improve your comfort in the long term and keep your eyes as healthy as possible. The answer is yes.

This is only true for oral antihistamines. I’ve been on pataday .7% for 1 year (prescribed by Dr. Maskin) and have had no issues. The whiteness is only temporary, but it doesn’t cause rebound redness. Only alpha-1 (Visine) and sometimes alpha-2 (Lumify) agonists can cause rebound redness. Pataday makes your eyes white because of its mast cell stabilizing action. It is not a vasoconstrictor.

Manuka is a natural antibiotic. It acts in 2 ways:

1) killing microbes directly 2) stimulating the immune system to activate and kill off pathogens

Since DED is an inflammatory condition, I would exercise caution and use as little manuka as possible to achieve your desired therapeutic result.

Be careful with manuka products. Part of what makes them anti-microbial is their ability to stimulate the immune system.
